Self Assessment Tax Return Questionnaire 20120/21 


To enable us to complete your Self Assessment Tax Return for the tax year ending 5th April 2021 please provide complete and accurate information on  the below questionnaire.

Section B: Income and Employment

If Yes, please list your employers' names, PAYE references', taxable pay and tax withheld. Where available, please provide a P60 (end of year tax certificate) OR P45 (if you left an employment during the tax year AND P11D (benefit-in-kind document – this may not be issued by your employer/previous employer until 6th July 2021).

Please note that if you traded as a Limited Company or Umbrella Company, you are NOT Self-Employed or a Sole Trader.

Please note that we will ask for the following evidance.


  • Income and Invoices.
  • Expenses.
  • Construction Industry Scheme (CIS) deductions if applicable.

Investments & Savings

If Yes, please list the banks/building societies, and the amount of interest received. Where available, please provide Statements of Interest. Please note ISA’s are exempt from tax and do not need reporting on your tax return. Please identify joint accounts and include 100% of the interest (we will report your 50% share).

Dividends

Land & Property

If Yes, please provide details of rents received and expenses paid. You may only claim property-related expenses that you have incurred and paid, (not expenses that tenants have paid) and relating to the period that the property was rented or on the market to be rented. Please provide your own records if applicable.

Tax Relief

Personal Pension Contributions

Please provide details of the pension provider, reference, net contribution and employer contribution (if applicable). Please also provide any statements you may have received from the pension provider. Net pay arrangements should NOT be reported here, as tax relief will already have been given at source.

Charitable Contributions

Please provide details of any charitable contributions you made during 2020/21 and the total amounts. Only include donations made personally; donations made by a limited company must not be included here.

Student Loan

If your answer is yes, and you have made repayments via your employer, please provide evidence of the payments made. You can obtain a statement from the Student Loans Company or they can give you this figure over the phone by calling 0300 100 0611.


As a Self Assessment taxpayer, loan repayments will be recalculated via your tax return to take into account dividend income. Loan repayments already made during the year will be taken into account.

Child Benefit

If Yes, please provide further details below of the amount of benefit received. If your gross income exceeds £50,000, you may be liable to repay a portion of the benefit via your Self Assessment tax return.

Should you be eligible for a tax refund, please provide your personal UK bank account details below. HMRC will usually repay any refund to you within four weeks. If the below is left blank a cheque will be issued.
